Leaving Employer Coverage? Medicare Enrollment Timeline

This is the basic flow for people who worked past 65 and delayed Medicare Part B because they had employer group health coverage.

60–90 days before employer coverage ends

Call Medicare On Main and confirm your target Medicare Part B start date. Do not guess on the date.

Create or access your SSA.gov account

Set up Login.gov or ID.me before you need to submit anything. This avoids last-minute login problems.

Get CMS-L564 completed by your employer

Your employer or HR department fills out the CMS-L564 form to verify your group health coverage.

Complete CMS-40B

This is the Part B enrollment form. Your requested Part B start date should match your transition plan.

Submit CMS-40B and CMS-L564

Submit through Social Security using the instructions they provide online, by mail, fax, or local office process.

Watch for your Part B confirmation

Do not enroll in a Medicare Supplement, Medicare Advantage, or Part D plan until the timing is clear.

Choose your Medicare plan

Once your Part B date is confirmed, Brian can help compare Medicare Supplement, Medicare Advantage, and Part D options.

Keep proof of employer coverage

Save employer letters, COBRA notices, coverage termination letters, and anything showing your group coverage dates.

Important: COBRA and retiree coverage can create Medicare timing problems. Call before relying on COBRA after age 65.
