Which Grand Junction Nursing Homes Take Medicare for Rehab?
Seven nursing homes in Mesa County are enrolled in Medicare for short-term skilled care. They are not equally busy. One of them handled more than half of the valley's Medicare rehab stays in 2024. Here is the list, what the numbers show, and how to choose when a discharge planner hands you the form.
The short answer
- Seven facilities: six in Grand Junction and Canyon View in Palisade, per CMS's October 2026 enrollment file.
- The Center at Foresight had 347 of the 621 Original Medicare rehab stays in 2024, about 56%.
- The average stay was about 28 days, which runs past the 20 days Medicare pays in full.
- You get to choose. With Original Medicare, the hospital must give you the list and respect your pick. With an Advantage plan, the network decides.
The seven Medicare nursing homes in Mesa County
CMS keeps a file of every skilled nursing facility enrolled in Medicare. The October 2026 release lists six in Grand Junction and one in Palisade. None are in Fruita or Clifton. A second CMS file counts how many Original Medicare rehab stays each one actually had. The latest year is fiscal 2024:
| Facility (CMS name) | Address | 2024 Medicare stays | Avg. days per stay |
|---|---|---|---|
| The Center at Foresight | 606 Foresight Cir E, Grand Junction | 347 | 30 |
| Larchwood Inns | 2845 N 15th St, Grand Junction | 96 | 26 |
| Eagle Ridge Post Acute | 2425 Teller Ave, Grand Junction | 54 | 31 |
| La Villa Grande Care Center | 2501 Little Bookcliff Dr, Grand Junction | 51 | 29 |
| Mantey Heights Rehabilitation and Care Center | 2825 Patterson Rd, Grand Junction | 32 | 24 |
| Canyon View Care Center | 151 E 3rd St, Palisade | 21 | 23 |
| Red Cliffs Post Acute | 2901 N 12th St, Grand Junction | 20 | 18 |

Two things stand out. Foresight is the rehab workhorse of the valley, with more Medicare stays than the other six combined. And the smaller numbers don't mean a place is worse. Several of these homes mostly care for long-term residents, and their rehab volume is small. The count tells you where Medicare patients have gone. It doesn't tell you quality. For that, use Medicare's Care Compare, which shows star ratings, staffing and inspection results for each one.
Why the average stay matters to your wallet
Across the seven facilities, Medicare paid for 17,612 days over 621 stays in 2024. That works out to about 28 days a stay. Under Original Medicare, days 1 through 20 cost you nothing. Days 21 through 100 cost $217 a day in 2026. A 28-day stay includes 8 of those days, or $1,736.
That bill is where your second layer of coverage shows up. Medigap Plans C, D, F, G, M and N pay the daily skilled nursing coinsurance in full. Plan K pays half and Plan L pays 75%, per Medicare.gov's Medigap chart. Plans A and B don't pay it. With a Medicare Advantage plan, the plan sets its own daily copays, which can start on day 1, and your yearly out-of-pocket maximum limits the total. The full day-by-day rules, including inpatient rehab hospitals, are in our post on how many days Medicare pays for rehab.
Who picks the facility: you, the hospital, or your plan?
When St. Mary's or Community Hospital is getting ready to discharge you, a discharge planner will bring up rehab. Federal rules for hospitals (42 CFR 482.43) spell out what happens next:
- You get a list. The hospital must give you a list of Medicare-participating skilled nursing facilities in the area you ask for. If your daughter lives in Palisade or Delta, ask for that area.
- You get to choose. The hospital must tell you about your "freedom to choose among participating Medicare providers" and "must not specify or otherwise limit" your options.
- Financial ties get disclosed. If the hospital has a financial interest in a nursing home it refers you to, the discharge plan must say so.
- Advantage members check the network. If you're in a Medicare Advantage plan, the hospital has to tell you to verify which facilities are in your plan's network.
In practice, your choice is limited by who has a bed that week. Call more than one. If your first choice is full, ask to be put on its list and whether you can transfer if a bed opens.

Medicare.gov says Advantage plans can waive the 3-day hospital stay rule, and that members "may be charged copayments during the first 20 days." Plans also usually require prior authorization for a skilled nursing stay. If you have a planned surgery, like a knee or hip replacement, handle three questions before the surgery date:
- Which of the seven Mesa County facilities are in my plan's network for 2027?
- What is my daily copay, and on which day does it start?
- Who requests the prior authorization, and how fast does the plan answer?
Open enrollment runs through December 7, so if those answers don't work for you, you can still change plans for January. Networks change from year to year, and our post on checking a 2027 network walks through how to look up a provider. Approvals don't always carry over to a new plan either. See whether prior authorization carries over.
Rehab is short-term. Long-term care is a different question.
Everything above is about skilled rehab after a hospital stay. If a parent needs help with bathing, dressing and meals for the long run, Medicare doesn't pay for it, even in the same building. That's long-term custodial care, and it's paid with savings, long-term care insurance or Medicaid. Our long-term care planning post for Grand Junction covers that side.
I've been helping people with Medicare for more than 22 years. The family that does best during a rehab stay is the one that knew the list, the costs and their plan's rules before the discharge planner knocked. That's what this page is for.
How we know all this: the Medicare On Main Data Desk frames every article with public data. Here that means CMS's Skilled Nursing Facility Enrollments file (October 2026), CMS's Medicare Post-Acute Care Utilization file for skilled nursing facilities (FY2024, fee-for-service Medicare), Medicare.gov's skilled nursing and Medigap pages, CMS's 2026 cost figures and the federal hospital discharge-planning rule at 42 CFR 482.43, all read October 26, 2026. Facility names are factual reporting, not endorsement or a quality ranking. This is education, not medical or legal advice. Compare plans with a licensed agent or Medicare.gov. We take no payment from any facility or carrier to feature it.
Frequently asked questions
What nursing homes in Grand Junction accept Medicare?
CMS's October 2026 enrollment file lists seven Medicare skilled nursing facilities in Mesa County: The Center at Foresight, Larchwood Inns, Eagle Ridge Post Acute, La Villa Grande Care Center, Mantey Heights Rehabilitation and Care Center and Red Cliffs Post Acute in Grand Junction, plus Canyon View Care Center in Palisade. Being enrolled means Medicare can pay for a covered rehab stay there. It doesn't mean a bed is open, so ask the hospital's discharge planner.
Can I choose which nursing home I go to for rehab after the hospital?
Yes, with Original Medicare. Federal discharge-planning rules (42 CFR 482.43) require the hospital to give you a list of Medicare-participating skilled nursing facilities in the area you ask about, to tell you that you are free to choose among them, and to disclose any facility the hospital has a financial interest in. With a Medicare Advantage plan, the hospital has to tell you to check the plan's network, because an out-of-network facility may not be covered.
How long does Medicare pay for a nursing home after a hospital stay?
Up to 100 days per benefit period after a qualifying inpatient stay of at least 3 days in a row. Days 1–20 cost $0 under Original Medicare. Days 21–100 cost $217 a day in 2026. In 2024, a Medicare rehab stay in a Mesa County nursing home ran about 28 days on average, so many people end up paying for a week or more of daily coinsurance.
Does Medicare Advantage cover rehab in a nursing home?
Yes, but on the plan's terms. Medicare.gov says Advantage members "may be charged copayments during the first 20 days," and plans can also waive the 3-day hospital stay rule. Plans usually require prior authorization and use a network, so before you pick a Grand Junction facility, ask the plan which ones are in network and get the approval in writing.
How do I compare Grand Junction nursing homes?
Use Medicare's Care Compare tool, which shows star ratings, staffing and inspection results for each Medicare-certified nursing home. Then call or visit. Ask about therapy hours per day, whether they have a bed, and, if you have a Medicare Advantage plan, whether they're in its network.
