Special Needs Plans (SNPs)

A Special Needs Plan (SNP) is a Medicare Advantage plan tailored to a specific group of people. A D-SNP serves people who have both Medicare and Medicaid (dual-eligible) and often includes very low costs and generous extra benefits. A C-SNP serves people with certain chronic conditions such as diabetes, heart disease, or COPD, with a plan designed around that condition.

Dual-eligible plans (D-SNP)

If you qualify for both Medicare and Medicaid, a D-SNP can offer $0 premiums, low or no copays, and extra allowances for groceries, over-the-counter items, and transportation, plus care coordination. If you're dual-eligible and not in a D-SNP, you may be missing hundreds of dollars in benefits a year. We check your eligibility and enroll you if it fits.

Chronic-condition plans (C-SNP)

A C-SNP is built around a specific chronic condition — its provider network, drug formulary, and extra benefits are tailored to people managing that diagnosis. If you have a qualifying condition, a C-SNP can coordinate your care and lower your costs better than a general plan.

How to qualify and enroll

D-SNP eligibility requires both Medicare and Medicaid (or a Medicare Savings Program); C-SNP eligibility requires a qualifying diagnosis confirmed by your doctor. How do I know if I qualify for a D-SNP?

You qualify if you have both Medicare and Medicaid (or your state's Medicare Savings Program). We can verify your status in Colorado or Utah and enroll you in a SNP if you're eligible.

What extra benefits do D-SNPs include?

Many include $0 premiums, grocery and over-the-counter allowances, transportation to appointments, dental and vision, and care coordination — well beyond a standard Advantage plan.

Can I switch to a SNP outside of fall enrollment?
