Medicare Advantage Plans Ending in 2027: County-by-County Lists for 18 States

Every fall some Medicare Advantage plans are not renewed, and the letter telling you so arrives in early October. These pages show the same thing from the federal plan file, for every county in the 18 states we are licensed in: which plans sold in 2026 are gone for 2027, how many people were on them, what remains, and what to do before December 7.

Sources: CMS CY2026 and CY2027 landscape files; CMS CPSC enrollment, September 2026. "Plans" = distinct general (non-Special-Needs) MA and MA-PD plan IDs sold anywhere in the state. Member counts include employer-group enrollees and omit counties where CMS suppresses counts under 11.

How to read these pages

  • Discontinued means the plan's ID has no row anywhere in the CMS 2027 file. Coverage ends December 31, 2026 and members get a non-renewal letter (42 CFR § 422.506).
  • Left the county means the plan continues in other counties but not this one. Members here get the same kind of letter.
  • New means a plan ID sold in the county in 2027 that was not sold there in 2026. A re-issued plan appears as gone and new; the letter says whether members are moved.
  • Special Needs Plans (dual-eligible, chronic-condition, institutional) are counted but not listed; their eligibility rules are different.
  • 2027 Star Ratings are not yet published; 2026 ratings are shown.

The three doors, in brief

  1. Another Medicare Advantage plan, chosen October 15 – December 7 so it starts January 1 (Medicare.gov). Doctors, prescriptions, in-network cap, then premium.
  2. Original Medicare + Medigap + a drug plan. A plan exit gives you a guaranteed-issue right to Medigap Plan A, B, D, G, K or L with no health questions, 60 days before to 63 days after the plan ends (Medicare.gov): November 1, 2026 through March 4, 2027.
  3. Original Medicare + a drug plan, no Medigap. No network and no annual cap on your 20% share.

Miss December 7 and a discontinued plan still gives you a Special Enrollment Period through February 28 (42 CFR § 422.62), with January spent in Original Medicare. Discontinued, or just changing? explains the two fall letters; Medigap after your plan ends covers the window.

How we know all this: the Medicare On Main Data Desk built these pages from the CMS CY2026 and CY2027 landscape files compared by Contract Plan ID, CMS's September 2026 county-level enrollment report, and the federal rules and Medicare.gov pages on non-renewal notices, enrollment periods and Medigap guaranteed-issue rights. Carrier names are factual reporting, not endorsement or criticism. This is education, not advice; confirm plans, costs and eligibility with a licensed agent or Medicare.gov. We take no payment from any carrier to feature a plan.

Frequently asked questions

How do I find out if my Medicare Advantage plan is ending in 2027?

Your plan must mail you a non-renewal notice at least 90 days before it ends, on or about October 2, 2026 for a December 31 end. These pages show the same information from the public CMS plan file: pick your state, then your county, and look for your plan's name and plan ID (the H-number on your card).

Where does this data come from?

From the CMS Medicare Advantage/Part D landscape files for 2026 and 2027 (the 2027 file was last updated 2026-09-22), compared plan number by plan number, and from CMS's September 2026 enrollment report by contract, plan, state and county. A plan counts as discontinued when its Contract Plan ID has no row anywhere in the 2027 file; a plan that left one county but continues elsewhere is marked separately.

Why does a plan show as gone and a similar one as new?

Carriers sometimes re-issue a plan under a new plan ID, often with different costs. Because we compare by ID rather than name, the old ID appears as gone and the new one as new. The non-renewal letter, or the plan, will say whether members are being moved.

What should I do if my plan is on the list?

Keep the letter. Choose 2027 coverage between October 15 and December 7: another Medicare Advantage plan, or Original Medicare with a stand-alone drug plan and, if you want it, a Medigap policy, which a plan exit lets you buy with no health questions from November 1, 2026 through March 4, 2027.
