Blue Shield Medicare Advantage Plans in San Diego for 2027
Blue Shield of California announced its 2027 Medicare lineup on October 1 without naming a county, a plan or a price. The CMS file and Blue Shield's own Summaries of Benefits fill that in for San Diego County: four $0 HMOs, two of them new with caps under $500. For a Sharp Direct Advantage member reading this as a possible landing spot, there is one caution that comes before any number.
The bottom line
- Four Blue Shield HMOs with drug coverage in San Diego County for 2027, all $0 premium, per the CMS 2027 landscape file: two new plans, Blue Shield Advantage (cap $499) and Blue Shield Value Advantage Plan (cap $500); 65 Plus (cap $1,750 → $1,800); and AdvantageOptimum Plan 1 (cap $4,200 → $4,400). Plus a Dual Special Needs Plan, renamed from TotalDual to Medicare + Medi-Cal Plan, premium $7 → $0.
- The sub-$500 caps are confirmed by Blue Shield's own 2027 Summaries of Benefits, which also show a $0 health plan deductible and a $300 Part D deductible that skips Tier 1 and Tier 2. A low ceiling says nothing about the copays under it; read the Summary of Benefits before you decide anything.
- The press release is a portfolio announcement. It lists plan types, "$0 premium plans," a "tailored and exclusive network" and a benefits menu footnoted "may not be offered in all our Medicare Advantage plans." Every county-level fact in this post comes from CMS and the Summaries of Benefits, not the release.
- Sharp caution: Sharp HealthCare's accepted-plans page lists Original Medicare with any Medigap, TRICARE for Life, Sharp Health Plan and UnitedHealthcare plans, "through the end of 2026." No Blue Shield Medicare Advantage plan is on it. Check your doctors by name in Blue Shield's 2027 directory before assuming anything.
- Blue Shield is one of 14 insurers with general plans on a county shelf that goes from 54 plans to 41 for 2027.
This is the seventh post in our series on Sharp Direct Advantage ending, and the first about a carrier that is arriving rather than leaving. This one is for the 19,697 Sharp Health Plan members in the county, and for anyone else shopping San Diego's 2027 shelf, who saw Blue Shield's October 1 announcement and wants to know what it actually means here. What the October 1 announcement says, and what it does not
Blue Shield's release (news.blueshieldca.com) is dated Oakland, October 1, 2026 and describes "its 2027 Medicare plans" statewide. Here is a fair summary of what is in it:

That is normal for a carrier announcement, and it is not a criticism. But it means that a San Diego reader cannot learn from the release which plans are sold here, what they cost or who is in the network. For that you need two other documents, and both are public.

Three observations, from the numbers only. First, Blue Shield's existing San Diego membership is small for a carrier its size: 1,931 on 65 Plus and 2,591 on AdvantageOptimum in September, against 7,440 on the dual plan and against 19,697 on Sharp Health Plan's plans. Second, the two continuing general plans drift upward in the usual 2027 way, caps up $50 and $200, drug deductibles up $50. Third, the two new plans are the story: $499 and $500 in-network caps are among the lowest in the county's file (the 2027 range in San Diego runs from $295 to $9,850), and they are new plan numbers with no history, no members and no Star Rating.
Checking the sub-$500 caps against Blue Shield's own documents
A number that low deserves a second source, so we read the 2027 Summaries of Benefits Blue Shield has posted for San Diego County. For Blue Shield Advantage (HMO): "Monthly plan premium ... $0," "Health plan deductible $0," and under "Annual maximum out-of-pocket amount": "This is the most you would pay for the year for in-network covered Medicare Part A and Part B services. $499." The Value Advantage Plan document carries the same wording with $500, and 65 Plus with $1,800. All three show the Part D "Stage 1: Annual deductible $300," with the note that it "doesn't apply to Tier 1 and Tier 2, covered insulin products and most adult Part D vaccines."
So the CMS figures are real. What they are not is a cost estimate. The cap is a ceiling on in-network Part A and B cost sharing; the copays under it, the referral rules, the medical groups in the network and the drug tiers decide what a year actually costs, and none of that is in the CMS file. We have deliberately not quoted a single copay from the Summaries of Benefits here, because a copay out of context is how people choose the wrong plan. Read the document for the plan you are considering, all of it, and put it next to your 2026 Evidence of Coverage.
The Sharp question, answered as far as the public record goes
If you are a Sharp Direct Advantage member, the plan you are leaving was built around Sharp's hospitals and medical groups, and the first thing you want to know about any new plan is whether your doctors are in it. Here is what can be said from public sources on October 8:
- Sharp HealthCare's accepted-plans page lists, under Medicare: Original Medicare, "All Medicare Supplement (Medigap) Plans," TRICARE for Life, Sharp Health Plan's individual and group plans, and a list of UnitedHealthcare individual plans plus a UnitedHealthcare group retiree plan. It carries the line "Sharp accepts these plans through the end of 2026."
- No Blue Shield of California Medicare Advantage plan appears on that page.
- Blue Shield's release describes "a tailored and exclusive network of providers" without naming one.
We are not saying a Blue Shield plan excludes Sharp doctors; a hospital's accepted-plans page is a statement about today, and networks are set plan by plan. We are saying that nothing public says it includes them, and that "Blue Shield" on a card is not "Sharp" on a card. The only reliable check is the one we described in the doctors post: look up each of your physicians by name in the 2027 directory for the specific Blue Shield plan, note which medical group the directory lists them under, and call the office to confirm they will take that plan in January. If the answer is no and keeping Sharp is the point, the paths that keep every Sharp doctor are the ones Sharp's own page lists: a UnitedHealthcare plan on that list, or Original Medicare with a Medigap policy, which has no network at all.

For a Sharp Direct Advantage member the doors have not changed. Door one is another Medicare Advantage plan on the county shelf, chosen October 15 – December 7 for a January 1 start (Medicare.gov), with the discontinued-plan Special Enrollment Period, December 8 – February 28, as a backstop that starts coverage on the first of the following month (42 CFR § 422.62). The four Blue Shield plans are four of the 41 choices behind that door; the full county list, what left and what arrived, is on the San Diego County reference page. Door two is Original Medicare plus Medigap with no health questions, November 1, 2026 – March 4, 2027 (Medicare.gov), plus a stand-alone drug plan; Blue Shield also sells Medicare Supplement plans and drug plans in California, and the release says so, though it names no premium. Door three is doing nothing, which lands you in Original Medicare alone.
Nothing moves a Sharp Health Plan member into a Blue Shield plan on its own. They are different insurers under different CMS contracts, and the October 2 letter describes options; it does not enroll you in any.
What I would do this week
- Download the Summary of Benefits for the Blue Shield plan that caught your eye and read the copay table, not just the cap line. The links above go straight to the San Diego County documents.
- Look up every doctor by name in Blue Shield's 2027 directory for that plan, note the medical group, and call the office. Do this before October 15, so the enrollment window is spent choosing, not searching.
- Run your prescriptions through Medicare.gov Plan Compare for the Blue Shield plans and for the stand-alone drug plans, so door one and door two are priced on the same page.
- Decide what the cap is worth to you. A $499 ceiling with a network you have to rebuild and a $2,900 ceiling you used to have with every Sharp doctor are different products, and a Medigap policy with no network is a third.
- Enroll between October 15 and December 7. A December 8 to 31 enrollment is the last one that still starts January 1.

Is the $499 out-of-pocket maximum on Blue Shield Advantage real?
It is what both the CMS file and Blue Shield's own 2027 Summary of Benefits for San Diego County say: "Annual maximum out-of-pocket amount ... This is the most you would pay for the year for in-network covered Medicare Part A and Part B services. $499." Value Advantage Plan says $500 in the same place. Those are unusually low ceilings, and a ceiling is not the whole story: the copays underneath it, the network and the referral rules are in the Summary of Benefits and Evidence of Coverage, and you should read those before assuming anything about what a year would cost.
Does Blue Shield Medicare Advantage cover Sharp doctors in San Diego?
Sharp HealthCare's own accepted-plans page lists Original Medicare with any Medigap plan, TRICARE for Life, Sharp Health Plan and UnitedHealthcare Medicare Advantage plans, and says Sharp accepts the listed plans "through the end of 2026." No Blue Shield of California Medicare Advantage plan is on that page. We are not saying Blue Shield excludes every Sharp doctor; we are saying nothing public says it includes them. A Sharp patient should look up each doctor by name in Blue Shield's 2027 directory for the specific plan and confirm with the office before enrolling.
Is Blue Shield 65 Plus changing for 2027?
Slightly, per the CMS file: the premium stays $0, the in-network cap moves from $1,750 to $1,800, and the Part D deductible from $250 to $300. It had 1,931 San Diego County members in September 2026. Copay changes, if any, are in the plan's Annual Notice of Change and 2027 Summary of Benefits, not in the CMS file.
Can a Sharp Direct Advantage member switch to a Blue Shield plan?
Yes, during the October 15 – December 7 Annual Enrollment Period for a January 1 start, or under the Special Enrollment Period a discontinued plan gives you, December 8 through February 28, with a start date on the first of the month after you enroll. Nothing moves you there automatically; Sharp Health Plan and Blue Shield are different insurers under different CMS contracts. Before choosing any plan for its cap or premium, confirm your doctors in that plan's 2027 directory, because the Sharp name is not on Blue Shield's plans or on Sharp's accepted-plans page.
