Is My Medicare Advantage Plan Discontinued in Utah for 2027?
If you got a letter in the last two weeks, this is the list to check it against. CMS's 2027 landscape file is public, and we compared every Utah Medicare Advantage plan in it, plan number by plan number, against the 2026 file. 5 plans are not offered anywhere for 2027. 2 are new. 12 continue but changed which counties they serve, which is a discontinuation for anyone in a dropped county. Here is every one, the county-by-county count for all 28 counties, and what your rights are if your plan is on the list.
The bottom line
- 5 Utah plans are gone for 2027: two HealthSpring plans, Select Health Medicare + Kroger, Humana Value Choice H5216-456 and Aetna Medicare Enhanced. If your card shows one of those, your coverage under it ends December 31.
- 7 more plans left some counties. Regence MedAdvantage + Rx Classic left eight, Select Health Medicare Essential left five, three Aetna plans left Iron County. In a dropped county that is a discontinuation with the same rights.
- The statewide shelf is 267 plan-county offerings, down from 280, but 179 are $0, one more than last year. 15 counties lost plans, 10 gained, mostly rural ones that picked up three UnitedHealthcare HMO-POS plans.
- Wayne County has no Advantage plan with drug coverage for 2027. 5 other counties have a single carrier.
- Your deadlines did not change. October 15 to December 7 for a January 1 start, a Special Enrollment Period to the end of February, and a 63-day Medigap guaranteed-issue window if you return to Original Medicare.
Utah had 490,235 people on Medicare in June 2026, and 251,606 of them, about 51%, were in a Medicare Advantage plan with drug coverage, per the CMS monthly enrollment file. Back in July, when Humana announced it would close plans nationally, we wrote that no county list existed and that the real answer would arrive in two places: your mailbox, by October 2, and CMS's 2027 landscape file, at the end of September. Both have now arrived. The file was posted at cms.gov, revised September 30, and under 42 CFR § 422.2263(a) plans "may begin marketing prospective plan year offerings on October 1." We read the whole Utah section and matched each plan's contract-plan number, the H-number and three digits printed on your membership card, against the 2026 file. Nothing below is estimated.
Two cautions. The file does not carry a 2027 Star Rating for any plan; CMS publishes those in October, so a 2027 rating you see before then is last year's. And Medicare On Main does not offer every plan available in your area. This is the list and what it means, not a ranking.
The 5 plans that are not offered for 2027
These plans appear in the 2026 Utah file and in no county of any state in the 2027 file. That is a non-renewal under 42 CFR § 422.506, and the plan was required to notify "each Medicare enrollee by mail at least 90 calendar days before" December 31.

Two notes on reading that table. HealthSpring, which had two general Advantage plans in seven Wasatch Front counties in 2026, has no Utah MA-PD plan at all for 2027; it still has a stand-alone drug plan in the state. And the Humana line is not the whole Humana story: a plan called Humana Value Choice (PPO) under a new number, H5216-471, appears in seven counties for 2027 at a $0 premium with a $5,000 in-network cap. Whether members of the old H5216-456 move into it automatically, or have to enroll, is in the letter, and this post explains the difference between a crosswalk and a non-renewal. Do not assume.
The 7 plans that continue but left some counties
This is the table people miss. A plan that is still sold in Salt Lake County can be discontinued in yours. The 2027 file lists each plan by county, so a county that disappears from a plan's list is a service-area reduction, and for the people who live there the plan ends December 31 with the same notice, the same Special Enrollment Period and the same Medigap right as a full non-renewal.

The net effect on carrier footprints: Regence BlueCross BlueShield of Utah goes from 12 counties to 4, all four on the Wasatch Front (Davis, Salt Lake, Utah, Weber). Select Health goes from 13 to 8, pulling out of Piute, Sanpete, Sevier and Wayne in central Utah and out of Morgan. Aetna Medicare goes from 15 to 5, with its Enhanced PPO not renewed in nine mostly rural counties and three plans leaving Iron County. What is new or expanding
It is not all subtraction. 2 plans are new to Utah, and 5 continuing plans added counties.
- Devoted Health: DEVOTED CHOICE GIVEBACK EXTRAS 014 UT (PPO) (H4473-014), $0 premium, $8,000 in-network cap, in 11 counties: Davis, Duchesne, Juab, Morgan, Rich, Salt Lake, Summit, Tooele, Utah, Wasatch, Weber.
- Humana: Humana Value Choice (PPO) (H5216-471), $0 premium, $5,000 in-network cap, in 7 counties: Cache, Davis, Morgan, Rich, Salt Lake, Utah, Weber.
- Devoted Health: DEVOTED CHOICE 001 UT (PPO) (H4473-001) added Duchesne, Juab, Rich, Utah, Weber.
- Devoted Health: DEVOTED CHOICE GIVEBACK 002 UT (PPO) (H4473-002) added Duchesne, Juab, Rich, Utah, Weber.
- UnitedHealthcare: AARP Medicare Advantage from UHC UT-0003 (HMO-POS) (H4604-003) added Beaver, Emery, Garfield, Juab, Kane, Millard, San Juan.
- UnitedHealthcare: AARP Medicare Advantage Essentials from UHC UT-4 (HMO-POS) (H4604-011) added Beaver, Emery, Garfield, Juab, Kane, Millard, San Juan.
- UnitedHealthcare: AARP Medicare Advantage Extras UT-7 (HMO-POS) (H4604-018) added Beaver, Emery, Garfield, Juab, Kane, Millard, San Juan.
The three UnitedHealthcare AARP HMO-POS plans moving into Beaver, Emery, Garfield, Juab, Kane, Millard and San Juan counties is the biggest single change for rural Utah. Each of those counties had four plans from two carriers in 2026 and has seven for 2027, still from the same two carriers. San Juan County, where our Monticello office is, is one of them. Devoted Health goes from 6 counties to 11, pushing into Utah County, Weber, Juab, Duchesne and Rich. Grand County, where our Moab office is, is unchanged: 8 plans from Humana and UnitedHealthcare, 5 at $0, for the 537 people here in an Advantage plan.

Three patterns in that table. First, the lowest cap in the state rose from $3,500 to $4,700 and the highest from $9,250 to $9,850; the highest premium rose from $135.00 to $158.00. Second, 15 of 28 counties now have two or fewer carriers, up from 14, and 5 have exactly one: Carbon, Piute, Sanpete, Sevier, Uintah. Third, Wayne County, which had one Select Health plan in 2026, has no general Advantage plan for 2027; its only Advantage plans are Dual Special Needs Plans for people who also have Medicaid. For everyone else in Loa, Bicknell and Torrey, 2027 Medicare means Original Medicare plus a stand-alone drug plan and, for most, a Medigap policy.
If your plan is on the list: your three doors
- Another Advantage plan, October 15 to December 7. Starts January 1. Check the 2027 network against your doctors and hospital, and the 2027 formulary against your prescriptions, before the extras. How we compare two plans, and which Utah hospital systems sit in which networks.
- The Special Enrollment Period, December 8 through the last day of February. 42 CFR § 422.62(b)(1) opens it when the organization "discontinued the plan in the area in which the individual resides." Per Medicare.gov, a choice made "from December 8–December 31" still begins January 1; a January choice begins February 1. December 31, not December 7, is the real wall.
- Original Medicare with a Medigap policy and a stand-alone drug plan. This is the door a discontinuation opens that nothing else does: a federal guaranteed-issue right to Medigap Plans A, B, D, G, K or L with no health questions. Medicare.gov says to apply "as early as 60 calendar days before the date your coverage will end, but no later than 63 calendar days after your coverage ends." Utah's birthday rule is a different thing, for people already in a Medigap plan; it does not apply to leaving Advantage. Utah has 8 stand-alone drug plans for 2027, from $5.30 to $238.00 a month, down from 10; pick by formulary, not premium.
And the fourth door, which is not really a door: do nothing, and on January 1 you are on Original Medicare with no drug plan and no Medigap policy, exposed to Part D's late enrollment penalty from the 64th day. Switching back to Original Medicare on purpose is a fine choice. Landing there by default is not.
If your plan is not on the list
Read your Annual Notice of Change anyway. 25 of the 27 plans on the 2027 list continue from 2026, and most of them changed something: a premium, a cap, a deductible, a drug tier. The county table above shows the 2027 MOOP range; if your plan's new cap is at the top of it, that is worth a second look even if the premium is still $0. The enrollment calendar is the same for you as for everyone else. Grand County's health profile, per CDC PLACES, is 33.3% of adults with high blood pressure, 28.2% with obesity and 11.2% with diabetes, which is the kind of profile where the drug list and the cap, not the gym membership, decide the year.
What I would do
Find your plan's contract-plan number on your card and check it against the two tables. If it is there, start with the Medigap door, because the guaranteed-issue window is the one thing you cannot get back once it closes, and compare the Advantage plans left in your county against it rather than only against each other. If it is not there, read the Annual Notice of Change with the county table beside it. Either way, doctors and drugs before extras, and a decision by December 7 so it starts January 1. Frequently asked questions
How do I know if my Medicare Advantage plan is being discontinued for 2027?
Your plan has to tell you in writing at least 90 days before it ends, so a non-renewal notice for a December 31 termination was due in your mailbox by October 2. The letter names the plan and says it will not be offered in 2027, or will no longer serve your county. You can also check the plan against the CMS 2027 landscape file: the 5 Utah plans that are gone entirely and the 7 that left particular counties are listed in this post, by plan name and contract-plan number, which is printed on your membership card.
Which Medicare Advantage plans are not offered in Utah for 2027?
Five plans from the 2026 file do not appear anywhere in the 2027 file: HealthSpring Premier (HMO-POS) from HealthSpring; Select Health Medicare + Kroger (HMO) from Select Health; Humana Value Choice H5216-456 (PPO) from Humana; Aetna Medicare Enhanced (PPO) from Aetna Medicare; HealthSpring True Choice (PPO) from HealthSpring. Twelve other plans continue but changed their county lists, including Regence MedAdvantage + Rx Classic leaving eight counties and Select Health Medicare Essential leaving Piute, Sanpete, Sevier, Wayne and Morgan. Medicare On Main does not offer every plan available in your area.
What happens if my Medicare Advantage plan is discontinued?
You keep Medicare. If you do nothing, you are returned to Original Medicare on January 1 with no drug coverage and no Medigap policy, which is the expensive default. You can join another plan October 15 to December 7 for a January 1 start, you get a Special Enrollment Period from December 8 through the last day of February, and you have a federal guaranteed-issue right to buy certain Medigap plans with no health questions, generally from 60 days before your coverage ends to 63 days after.
Is Humana leaving Medicare Advantage in Utah in 2027?
No. Humana offers plans in 23 Utah counties in both the 2026 and 2027 files. One Humana PPO, Value Choice H5216-456, is not renewed in its 12 counties, and a plan with the same name under a new number, H5216-471, appears in 7 counties at a $0 premium with a $5,000 out-of-pocket maximum. A same-name plan under a new number is not automatically your plan; your letter says whether you move on your own or have to act.
How many Medicare Advantage plans are there in Utah for 2027?

Can I get a Medicare Supplement if my Advantage plan is discontinued in Utah?
Yes, with no health questions, if you return to Original Medicare. Federal law gives you a guaranteed-issue right to Medigap Plans A, B, D, G, K or L (C or F only if you were eligible for Medicare before 2020) when your plan leaves Medicare or stops serving your county.
