Which Utah Counties Does Select Health Medicare Cover?
13 of Utah's 29 counties for 2026, and not the 16 you might guess. Here is the county-by-county map of Select Health's five Medicare plans, taken from the CMS landscape file rather than a brochure, what "service area" means when you enroll or move, and the counties where the hospital is Intermountain's but the insurer is not on the shelf.
The bottom line
- 13 counties: Cache, Davis, Iron, Morgan, Piute, Salt Lake, Sanpete, Sevier, Summit, Utah, Washington, Wayne and Weber. That is the CMS landscape file's list for contract H1994 and, word for word, the list on page 2 of Select Health's 2026 provider directory. The other 16 counties have no Select Health Medicare plan this year.
- Five plans, all HMOs. Three Medicare Advantage drug plans (two along the Wasatch Front, one for central and southern Utah), one dual-eligible HMO D-SNP, and one plan without drug coverage. No PPO. Every drug plan carries a $0 plan premium and a maximum out-of-pocket between $5,500 and $6,325; every row carries a 4.5-star overall rating.
- The footprint is where the people are. The 13 Select Health counties hold 178 of Utah's 280 Medicare Advantage drug-plan offerings for 2026, so Select Health competes on the crowded shelves and sits out most of the thin ones — except Wayne County, where it is the only Medicare Advantage drug plan at all.
- Not Moab, not Monticello. Grand and San Juan counties each have 2 carriers and neither is Select Health. Medicare.gov's rule: "Live in the service area of the plan you want to join." Where your hospital is does not change where your address is.
- 2027 is not in this article. CMS publishes the 2027 landscape on October 1, and service areas can change year to year. Everything here is the 2026 plan year, as filed.
Here is why this question needs a table rather than a yes or no. Select Health is Intermountain Health's insurance company, and Intermountain's hospitals are in most of Utah, so people assume the Medicare plan follows the hospital. It does not. A Medicare Advantage plan is filed county by county with CMS, and the 2026 file shows Select Health in 13 counties, with different plans reaching different counties. The five plans and the counties each one reaches
The CMS landscape file lists every approved plan by county. Filtered to Utah and to Select Health, it holds 5 plans under one contract, H1994, spread across 32 county-rows. Premium is the monthly consolidated (Part C plus Part D) premium as filed; the maximum out-of-pocket is the in-network figure; the deductible is the annual Part D drug deductible.

Two things stand out. First, the two "Essential" plans are different products with different service areas, deductibles and out-of-pocket limits, even though they share a name: H1994-001 is the Wasatch Front and Cache Valley plan, H1994-017 is the central and southern Utah plan, and the only counties both a Wasatch plan and the southern plan touch are Iron and Washington, through the Kroger plan. Second, the two plans that are not standard drug plans, the D-SNP and the no-drug HMO, reach only the 4-to-6 biggest counties. In Piute, Sanpete, Sevier and Wayne, "Select Health Medicare" means exactly one plan.
County by county: how much of the shelf is Select Health
The table below puts each Select Health county next to its whole 2026 shelf of Medicare Advantage drug plans, from the same CMS file. "Share" is Select Health's drug plans as a percentage of the county's total.

Read the share column as a description of the county, not a verdict on the carrier. In Salt Lake, Davis, Weber, Utah and Morgan, Select Health is two plans among two dozen from six or seven carriers, and a shopper there is choosing a network first and a logo second. In Wayne County it is the entire Medicare Advantage market: one plan, one carrier, 1 of 1 at a $0 premium, with a $5,700 maximum out-of-pocket. Sanpete and Sevier are two-carrier counties where the alternative is a single national carrier. That is the whole shelf, and the decision there is really Medicare Advantage versus Original Medicare with a supplement, which is a different article: this one.

Beaver, Box Elder, Carbon, Daggett, Duchesne, Emery, Garfield, Grand, Juab, Kane, Millard, Rich, San Juan, Tooele, Uintah and Wasatch. That is more than half the map and, by population, much less than half the state. A few of these are large shelves where the absence is a real gap in the comparison: Tooele (15 plans), Wasatch (13 plans), Box Elder (12 plans), Duchesne (9 plans), Grand (8 plans). Most of the rest are the two-carrier counties of eastern and southern Utah, where the shelf is thin for everyone. Daggett has no Medicare Advantage drug plan from any carrier in the 2026 file.
Grand and San Juan sit in the second group. Moab and Monticello each have 2 carriers, 8 plans in Grand County and 4 in San Juan, and Select Health is in neither. If your cardiologist is at Intermountain Medical Center in Murray or your surgeon at St. George Regional, that is a network question for the plans that are filed in your county, not a reason to look for a plan that is not.
Where the hospital is Intermountain's and the insurer is absent
The assumption I hear most is "Intermountain runs the hospital here, so Select Health must be here." The CMS file says otherwise in three counties. Cross-referencing the CMS Hospital General Information file with the landscape file:

The lesson runs the other way too. Select Health being in your county tells you nothing about whether your particular clinic is in its network; that is a directory check, plan by plan, which our Utah hospital-networks post walks through. The county is the gate. The network is the room behind it.
What "service area" means when you enroll, and when you move
Medicare.gov lists three conditions for joining a Medicare Advantage plan, and the second is geography: "Live in the service area of the plan you want to join." The service area is the county list above, by permanent residence, which is why a Park City household with a second home in St. George picks a plan by where it is registered to vote and gets mail, not by where it winters; we covered that in the Park City post.
Leaving the service area is the case that catches people. Suppose you retire from Sandy to Moab. Grand County is outside every Select Health service area, so the plan cannot follow you, and Medicare.gov gives you a Special Enrollment Period with a clock: "Your chance to switch begins when you move and continues for 2 full months after you move. If you tell your plan before you move, your chance to switch plans begins the month before the month you move and continues for 2 full months after you move." Miss the window and the default is not a different plan. It is no plan: "If you move outside your old plan's service area and don't join a new Medicare Advantage Plan during this Special Enrollment Period, you'll be enrolled in Original Medicare when you're dropped from your old Medicare Advantage Plan." Original Medicare with no drug coverage and no yearly out-of-pocket limit, until you fix it.
Because every Select Health Medicare plan is an HMO, the same geography applies to care while you still live in the service area. Medicare.gov's HMO page limits out-of-network coverage to "Emergency care, Out-of-area urgent care, Temporary out-of-area dialysis," and warns that otherwise "you may have to pay the full cost." A Richfield member on the southern Essential plan who wants a specialist in Salt Lake needs that specialist to be in the plan's directory, or a referral the plan approves; the county line is not the network line, and neither is the Intermountain logo.
What I would do
If you live in one of the 13 counties, treat Select Health as one row on the shelf and run the same three checks on it that you run on the others: your doctors in the 2027 directory by name, your prescriptions on the 2027 formulary by tier, and the maximum out-of-pocket against a bad year. The 4.5-star rating is public and worth noting; it is not a substitute for those checks. If you live in one of the 16 counties, stop looking for it, and make the decision the shelf actually offers, which in Grand and San Juan is two carriers' Advantage plans against Original Medicare with a Medigap policy. And if you are moving in either direction across that line this year, put the move date on the calendar next to October 15 and December 7, because the moving window and Open Enrollment can overlap, and the moving window is the shorter one.
One more local note. In Grand County, 33.3% of adults live with high blood pressure and 11.2% with diabetes; the care for both is mostly primary care and pharmacy, both of which exist in Moab on every plan filed here. The specialty care leaves the county under every plan too. The right question for a Moab reader is not which carrier is missing, but which of the plans that are present covers the trip north.

Which counties does Select Health Medicare cover?
For 2026, 13 Utah counties: Cache, Davis, Iron, Morgan, Piute, Salt Lake, Sanpete, Sevier, Summit, Utah, Washington, Wayne and Weber. That is the list in the CMS landscape file for contract H1994 and the list printed on page 2 of Select Health's own 2026 provider directory. The other 16 counties, including Grand, San Juan, Tooele, Box Elder, Wasatch and Carbon, have no Select Health Medicare plan this year.
Is Select Health Medicare available in St. George?
Yes. Washington County is in the service area of 2 Select Health drug plans for 2026 (H1994-022 and H1994-017) and the dual-eligible plan, out of 12 Medicare Advantage drug plans in the county from 5 carriers. Cedar City (Iron County) and Richfield (Sevier County) are in the service area too.
Does Select Health have Medicare Advantage plans in Moab or Grand County?
No. Grand County's 8 Medicare Advantage drug plans for 2026 come from 2 carriers, and Select Health is not one of them; San Juan County's 4 plans come from the same two. Medicare.gov's rule is that you must "Live in the service area of the plan you want to join," so a Moab or Monticello address cannot enroll in a Select Health Medicare plan, whichever hospital you use.
Is Select Health Medicare an HMO or a PPO?
Every one of Select Health's 2026 Utah Medicare plans is an HMO — three drug plans, one dual-eligible HMO D-SNP and one plan without drug coverage. There is no Select Health PPO in the file. On an HMO, Medicare.gov says out-of-network care is covered only for "Emergency care, Out-of-area urgent care, Temporary out-of-area dialysis," and otherwise "you may have to pay the full cost," so the plan's directory is the network.
Is Select Health part of Intermountain Health?
Yes. The CMS landscape file lists the parent organization of contract H1994 as "Intermountain Health Care, Inc.," and Select Health's own site refers to "our parent company, Intermountain Health." That does not make every Intermountain hospital a Select Health county: Heber Valley, Delta Community, Fillmore Community and Garfield Memorial are Intermountain-named hospitals in Wasatch, Millard and Garfield counties, none of which has a Select Health Medicare plan for 2026.
What happens to my Select Health plan if I move to a county it doesn't cover?
You get a Special Enrollment Period, and a deadline. Medicare.gov: "Your chance to switch begins when you move and continues for 2 full months after you move. If you tell your plan before you move, your chance to switch plans begins the month before the month you move." Miss it and, in Medicare.gov's words, "you'll be enrolled in Original Medicare when you're dropped from your old Medicare Advantage Plan" — with no drug plan and no yearly out-of-pocket limit until you choose something.
