Medicare Advantage Plans in Salt Lake City, Utah for 2027

CMS's 2027 landscape file is public, and Utah's biggest Medicare market has a slightly shorter, noticeably pricier-to-use shelf: 22 Advantage plans with drug coverage instead of 24, 15 at $0 instead of 17, in-network caps of $4,700 to $9,850 instead of $3,500 to $9,250, and 11 plans charging the full $700 drug deductible. Every plan with its own 2026 numbers beside it, the four that are gone, the two that are new, and the calendar through December 7.

The bottom line

  • 22 plans, 6 carriers. Aetna Medicare, Devoted Health, Humana, Regence BlueCross BlueShield of Utah, Select Health and UnitedHealthcare. HealthSpring, which had two Salt Lake County plans in 2026, has none in Utah for 2027.
  • 15 of 22 have a $0 premium. All four discontinued plans were $0 and only two $0 plans are new. The highest premium rose from $135 to $158; one premium fell sharply, Aetna Value Plus from $37.60 to $6.50.
  • Out-of-pocket caps run $4,700 to $9,850. Thirteen of the twenty continuing plans raised their cap. The county's lowest cap in 2026, $3,500, belonged to a plan that is gone; the lowest for 2027 is Humana Total Complete HMO at $4,700.
  • Drug deductibles jumped. 11 plans now charge the $700 federal maximum and five more charge $685; in 2026 the range was $200 to $615. The Select Health HMO went from $200 to $700.
  • 4 plans discontinued, 2 new. Star Ratings are not in the file yet; CMS publishes 2027 ratings in October. Enrollment runs October 15 to December 7.

Salt Lake County is where Utah's Medicare market is. It had 166,854 people on Medicare in June 2026, and 96,718 of them, about 58%, were in a Medicare Advantage plan, per the CMS enrollment file; another 43,721 paired Original Medicare with a stand-alone drug plan. When we counted the 2026 shelf in August it was the widest in the state, 24 plans from 7 carriers with caps from $3,500 to $9,250. The 2027 file, which CMS posted at the end of September, is the first look at what changed, and plans may market it from October 1 under 42 CFR § 422.2263.

Two cautions before the table. The landscape file carries premiums, caps, deductibles and plan types; it does not carry copays, networks, formularies or extras, and the 2027 Star Rating column is blank in this release. And a plan that keeps its name can still change underneath you: the arrows in the table are the point, not the names. If you are in a Salt Lake County plan today, your Annual Notice of Change says what your plan did; this table says what everyone else's did.

The 2027 Salt Lake County shelf: all 22 plans, with their 2026 numbers

Medicare Advantage plans with drug coverage ("MA-PD" in the file), sorted by carrier. Premium is the consolidated Part C plus Part D monthly premium, on top of your Part B premium. MOOP is the in-network maximum out-of-pocket for the year. The drug deductible is the plan's Part D deductible; for 2027 the federal maximum is $700 and every plan caps your drug spending at $2,400. A dash means the plan did not exist in the 2026 file.

Read the arrows. Thirteen caps went up, four went down (Humana Total Complete, Humana Value Choice H7617-032, the AARP Essentials HMO-POS, and the AARP Giveback PPO, which dropped $1,750), three held. Every Aetna plan raised its cap by $1,000 or more. Humana Value Choice H7617-032 cut its cap by $800 and is now one of two $0 plans at $5,000, the other being the new Humana Value Choice H5216-471. On premiums, Regence raised both of its plans, Humana raised all three of its paid plans, and Aetna Value Plus cut its premium by more than $31 while raising its cap by $1,000, a trade that only makes sense once you know how much care you use. On deductibles, the Select Health HMO's move from $200 to $700 is the biggest single change on the page for anyone on a brand-name drug; the AARP Giveback PPO went the other way, from $600 to $215.

The 4 plans that are discontinued for 2027

Four 2026 Salt Lake County plans are not in the 2027 file in any county in the country. All four were $0-premium plans, and one of them, HealthSpring Premier, carried the lowest out-of-pocket cap in the county. If you are in one of them, the statewide post walks through the three doors a non-renewal opens.

Carrier2026 plan2026 premium / MOOPWhat happened
HealthSpring HealthSpring Premier (HMO-POS) (H0439-024) $0 / $3,500 Not offered anywhere for 2027 (non-renewal); it had the county's lowest 2026 cap
HealthSpring HealthSpring True Choice (PPO) (H7849-029) $0 / $5,300 Not offered anywhere for 2027 (non-renewal)
Humana Humana Value Choice H5216-456 (PPO) (H5216-456) $0 / $5,800 Not offered anywhere for 2027 (non-renewal)
Select Health Select Health Medicare + Kroger (HMO) (H1994-022) $0 / $6,325 Not offered anywhere for 2027 (non-renewal)

Two of the four have a near-namesake that continues. Humana Value Choice H5216-456 is gone, but two different plans called Humana Value Choice remain, H7617-032 (continuing) and H5216-471 (new), both at $0 with a $5,000 cap and a $700 deductible. Select Health Medicare + Kroger is gone, and Select Health Medicare Essential continues as the carrier's only Salt Lake County plan. A same-name plan is not the same plan; the contract-plan ID on your card is what to match.

Plans without drug coverage, and Special Needs Plans

The MA-PD table is not the whole file. Salt Lake County also has 6 Medicare Advantage plans with no Part D coverage for 2027, down from 7; these are built for people with drug coverage from somewhere else, most often the VA or TRICARE For Life, and a reader with neither should not join one, because a year without creditable drug coverage starts the Part D late enrollment penalty clock.

There are 18 Special Needs Plans in the county for 2027, up from 17, for people with both Medicare and Medicaid, with specific chronic conditions, or living in an institution. Each has its own eligibility test, and none of them appear in the table above.

Davis, Utah, Weber, Tooele and Summit counties

The Wasatch Front is not one market. Each county has its own list, and 2027 moved them differently.

  • Salt Lake, Davis and Utah counties are now tied at 22 plans from the same six carriers, with the same $4,700 to $9,850 cap range. Utah County is the only one of the three that grew, from 21 to 22. Our Utah County post has the 2026 baseline.
  • Weber held at 19, with 13 at $0. The Ogden and Layton post has the 2026 list.
  • Tooele and Summit lost the most, 15 to 11 and 16 to 12, as Aetna and Regence left both counties and HealthSpring left Tooele. Park City readers in particular have a shorter list than they had.

If you are going back to Original Medicare

A discontinued plan is one of the few events that lets a Utahn buy a Medigap policy without health questions outside the first six months on Part B, and Utah's birthday rule, which does not help someone leaving Advantage, is not needed for it. The window is 63 days from the day coverage ends. Pair the Medigap policy with one of Utah's eight stand-alone Part D plans for 2027, which run $5.30 to $238.00 a month. For a Salt Lake County reader whose doctors are at University of Utah Health, Intermountain and a private group all at once, the no-network route deserves a serious look beside the 22 plans above.

What the list does not tell you

Three things decide most of these choices, and none of them are in the landscape file.

  • Whether your doctors are in the 2027 network. University of Utah Health, Intermountain Medical Center and the LDS, Alta View and St. Mark's campuses each have to be checked against each plan you are considering, for 2027 specifically. Our University of Utah Health post and the Utah hospital-networks post explain how; the 2027 directories are what to check.
  • Whether your prescriptions are covered, and at what tier. The file gives the deductible, and for 2027 that number is $700 or $685 on 16 of the 22 plans. It does not give the formulary, the tiers, or which drugs need prior authorization. Which pharmacies are preferred matters almost as much.
  • The Star Rating. Blank in this release. CMS publishes 2027 ratings in October, and a contract that moves up or down a half star is worth noticing. We will update this post when they land.

Salt Lake County's health profile is the reminder of why the cap and the formulary outweigh the extras. Per CDC PLACES, 28.1% of adults here live with high blood pressure, 29.9% with obesity and 8.7% with diabetes, conditions that mean specialists, imaging and prescriptions every year, not just in a bad one. How to compare two plans gives the order to check things in.

The calendar from here

  • Now: plans can market 2027 offerings, and Medicare.gov's Plan Finder shows them. Your Annual Notice of Change should already be in hand; here is how to read it.
  • October 15 to December 7: enrollment. A plan you join in this window starts January 1, per Medicare.gov.
  • December 8 through the last day of February: the Special Enrollment Period for people whose plan was discontinued. A choice made by December 31 still begins January 1.
  • January 1 to March 31: the Medicare Advantage Open Enrollment Period, one change for anyone already in an Advantage plan.
  • January 1: if you were in a discontinued plan and did nothing, you are on Original Medicare with no drug plan and no Medigap policy.

What I would do

If your plan is on the second table, start with the Original Medicare and Medigap door, because the 63-day guaranteed-issue window is the thing you cannot get back, and compare the 22 plans above against it rather than only against each other. If your plan continues, find its row, read its three arrows, and then read your Annual Notice of Change for the copays the file does not carry. A $0 plan with a $700 drug deductible and a $7,150 cap is a different product from a $0 plan with a $215 deductible and the same cap, and the mailers will not tell you which is which. Then check the doctors, then the drugs, then decide.

Frequently asked questions

How many Medicare Advantage plans are in Salt Lake County for 2027?

22 Medicare Advantage plans with drug coverage, from 6 carriers, according to the CMS 2027 landscape file published at the end of September. That is down from 24 in 2026, and it is still the most of any Utah county, tied with Davis and Utah counties. 15 of the 22 have a $0 premium. The county also has 6 Advantage plans with no drug coverage and 18 Special Needs Plans, counted separately.

Which insurance companies offer Medicare Advantage plans in Salt Lake City?

For 2027: Aetna Medicare (4 plans), Devoted Health (3), Humana (7), Regence BlueCross BlueShield of Utah (2), Select Health (1) and UnitedHealthcare (5). HealthSpring, which sold two Salt Lake County plans in 2026, has no Medicare Advantage drug plan anywhere in Utah for 2027. Humana has the most plans on the shelf; UnitedHealthcare has the most HMO-POS plans; Select Health is down to one.

Which Medicare Advantage plans are discontinued in Salt Lake County for 2027?

Four 2026 plans are not in the 2027 file in any county: HealthSpring Premier (HMO-POS), HealthSpring True Choice (PPO), Humana Value Choice H5216-456 (PPO) and Select Health Medicare + Kroger (HMO). All four were $0-premium plans. Anyone in one should have received a non-renewal notice by early October and has until December 7 to pick a plan for January 1, plus a Special Enrollment Period through the end of February and, for the Medigap route, a 63-day guaranteed-issue window.

What is the out-of-pocket maximum for Medicare Advantage plans in Utah for 2027?

In Salt Lake County it runs from $4,700 (Humana Total Complete HMO) to $9,850 (Devoted Choice Giveback 002 PPO) for in-network care, up from $3,500 to $9,250 in 2026. Thirteen of the twenty continuing plans raised their cap; four lowered it, including the AARP Giveback PPO from $8,900 to $7,150; three held steady. The county's lowest 2026 cap, $3,500, belonged to a HealthSpring plan that is gone.

Do Medicare Advantage plans in Salt Lake City have a $0 premium?

15 of the 22 plans for 2027 do, two fewer than in 2026, because the four discontinued plans were all $0 and only two $0 plans arrived. The other seven range from $6.50 (Aetna Medicare Value Plus PPO, down from $37.60) to $158 (Regence MedAdvantage + Rx Enhanced PPO, up from $135). A $0 premium is not free: you still pay the Part B premium and the plan's copays up to its cap.

When can I change my Medicare Advantage plan in Utah for 2027?

October 15 to December 7, for a January 1 start. Plans may market 2027 offerings from October 1, and Medicare.gov's Plan Finder shows them from the same date. If your plan was discontinued you also get a Special Enrollment Period from December 8 through the end of February, and anyone already in an Advantage plan gets one more change from January 1 to March 31.
