Grand Junction and Mesa County · Same-day surgery for 2027
Outpatient Surgery Cost on Medicare in Grand Junction, 2027
On a 2027 Advantage plan in Mesa County, the facility copay for same-day surgery runs $175 to $400 at a surgery center and up to $280 to $450 in a hospital's outpatient department. Where the procedure is booked can change your share by $35 to $105.
The short answer
- Original Medicare: 20% of the approved amount after the Part B deductible ($283 in 2026), to the facility and to each doctor, with no yearly limit.
- Advantage, surgery center: a flat $175 to $400 facility copay across Mesa County's 13 general plans for 2027.
- Advantage, hospital outpatient: a top copay of $280 to $450, and 11 of the 13 plans charge more here than at a surgery center.
- Every plan requires prior authorization for both, and none uses a deductible.
Two prices, depending on where the surgery is booked
Cataract removal, a knee scope, a hernia repair, a colonoscopy with a polyp taken out: most of these happen as same-day surgery. Medicare.gov points out that you may get a choice between an ambulatory surgical center (a freestanding surgery center) and a hospital outpatient department, and that what you pay may differ. Under Original Medicare, both cost 20% of the approved amount after the Part B deductible, per Medicare's surgery center and outpatient hospital pages. The approved amount itself is usually higher at a hospital.
Advantage plans swap the 20% for flat copays, and they price the two settings separately. I matched CMS's 2027 plan benefit file to the 13 general plans sold in Mesa County. Here's what each one files for the facility.

Premium is $0 unless shown. Listing a plan is reporting, not a recommendation, and we do not offer every plan available in your area.
Most plans file a range starting at $0 for both settings. The $0 end covers services such as a screening colonoscopy, which Medicare treats as preventive. For an actual procedure, plan on the top figure unless the plan's Summary of Benefits lists your procedure lower. The two Select Health plans also file 20% coinsurance on some hospital outpatient services, so ask them which applies to yours.
The surgery center usually wins, by $35 to $105
On 11 of the 13 plans, the surgery-center copay is lower than the hospital outpatient copay. The widest gap is on Select Health Medicare Essential ($175 against $280) and HumanaChoice H5216-223 ($225 against $325). The narrowest is HealthSpring Preferred ($390 against $425). Two plans charge the same at both: HealthSpring True Choice at $345 and Select Health Medicare Veterans at $400.
That gap is one surgery's worth. Someone who has cataracts done one eye at a time pays the copay twice, so the setting matters twice. Our cataract surgery post covers what Medicare pays for the lenses.
How to tell which one you're booked into
Same-day surgery inside a hospital is billed as hospital outpatient care. A freestanding surgery center bills as an ambulatory surgical center. Some surgery centers are owned by hospitals or physician groups, so the name on the door doesn't always settle it. When the surgeon's scheduler calls, ask three things:
- "Is this billed as a hospital outpatient department or an ambulatory surgery center?" That decides which line of the table applies.
- "Is the facility in my plan's 2027 network?" These are in-network amounts. On an HMO, a facility outside the network generally isn't covered for planned surgery. On a PPO it costs more. Our network-check post walks through it.
- "Has the prior authorization been approved?" CMS's file marks authorization as required for both settings on all 13 plans.
The facility copay isn't the whole bill. The surgeon and the anesthesia provider bill separately, and plans usually apply a specialist or physician copay to that. Our imaging copay post covers the pre-op scan side.

Book a call →Observation: the overnight stay that is still outpatient
If you stay in a hospital bed overnight after a procedure without a formal admission order, you're usually under observation. Medicare.gov lists observation with outpatient hospital services, even overnight. So the daily inpatient copays in our hospital copay post don't apply. The observation copay does.
Every general plan in Mesa County files observation as one charge per stay for 2027, from $280 (Select Health Medicare Essential) to $465 (Humana Essentials Plus Giveback). On two Humana plans, Value Choice and USAA Honor Giveback, the observation copay is lower than the plan's hospital outpatient copay. Eleven of the 13 plans also require authorization for observation; the two Select Health plans don't.
When Original Medicare plus a Medigap plan comes out ahead
Under Original Medicare alone, outpatient surgery has no flat price: 20% of the facility's approved amount plus 20% of each doctor's, and no yearly limit. Medicare.gov does cap the copay for any single hospital outpatient service at the inpatient deductible, $1,736 in 2026. That limit applies to each service, not to the whole visit.
A Medigap Plan G pays that 20% once you've met the Part B deductible, so after the deductible a planned surgery costs you nothing beyond the monthly premium. It also skips the prior authorization and network questions. If you expect several procedures in 2027, compare the copays above, times the number of procedures, against a year of Medigap premiums. Our Medigap-or-Advantage post walks through that trade. Every copay above also counts toward the plan's yearly cap, $4,900 to $9,850 for 2027.
The order I'd check things in
Start with whether your surgeon and their surgery center are in the plan's network. Then look at the surgery-center copay, then the cap. A $50 difference in facility copay matters less than a surgeon who isn't in network. Open Enrollment runs through December 7, and any plan change starts January 1.
How we know all this: the Medicare On Main Data Desk frames every article with public data. Here that means CMS's 2027 plan benefit and landscape files and Medicare.gov's surgery, surgery center and outpatient hospital pages, all read November 7, 2026. Benefits can differ in a plan's final documents, so confirm them in its Summary of Benefits and Evidence of Coverage. Carrier names are factual reporting, not endorsement. This is education, not advice. Compare plans with a licensed agent or Medicare.gov. We take no payment from any carrier to feature a plan.
Frequently asked questions
How much does Medicare Advantage pay for outpatient surgery?
The plan pays the facility and you pay a flat copay. In Mesa County for 2027, the 13 general plans charge $175 to $400 at an ambulatory surgery center and up to $280 to $450 at a hospital outpatient department, per CMS's plan benefit file. The surgeon's bill can carry its own copay.
Is outpatient surgery cheaper at a surgery center or a hospital?
On most Mesa County plans, yes, the surgery center costs less. Eleven of the 13 charge $35 to $105 less there than at a hospital outpatient department. HealthSpring True Choice and Select Health Medicare Veterans charge the same top amount at both.
What does Original Medicare pay for outpatient surgery?
After the Part B deductible ($283 in 2026), you pay 20% of the Medicare-approved amount to the surgery center or hospital and 20% to each doctor, with no yearly cap. Medicare.gov says the copay for a single hospital outpatient service can't exceed the inpatient deductible, $1,736 in 2026.
Do I need prior authorization for outpatient surgery on Medicare Advantage?
On every general Mesa County plan for 2027, yes. CMS's file marks authorization as required for both surgery center and hospital outpatient services on all 13 plans. Your surgeon's office normally sends the request; confirm it's approved before the date.
Is observation status inpatient or outpatient?
Outpatient. Medicare.gov lists observation with outpatient hospital services, even if you stay overnight. In Mesa County every general plan bills observation as one charge per stay for 2027, from $280 to $465.
