Grand Junction and Mesa County · Rehab after the hospital, 2027

Do Grand Junction Medicare Plans Waive the 3-Day Rule?

Original Medicare won't pay for rehab in a nursing home unless you spent three days as a hospital inpatient first. All 13 general Advantage plans in Mesa County drop that rule for 2027. Nine need no inpatient day at all. Aetna's four need one.

The short answer

  • Original Medicare: a skilled nursing stay is covered only after at least 3 days as an admitted inpatient. Observation nights don't count.
  • Mesa County Advantage plans, 2027: all 13 general plans accept less. Nine require 0 inpatient days; the four Aetna plans require 1.
  • The daily cost: $0 for days 1–20 on 11 plans ($10 on two), then $218 or $221 a day for days 21–100.
  • The catch: every plan requires approval, and the nursing home has to be one the plan will pay.

Why the 3-day rule trips people up

Medicare.gov spells out the rule: Part A covers a skilled nursing facility only after a "qualifying inpatient hospital stay" of three days in a row. The day you're admitted counts. The day you leave doesn't. Time spent in the emergency room or under observation before the admission order doesn't count either, "even if you're there overnight."

That last part is where families get hurt. Someone falls, spends two nights at St. Mary's under observation, then needs two weeks of therapy before going home. On Original Medicare, that nursing home stay isn't covered, because the hospital never admitted them. Observation is outpatient care, and Medicare.gov's inpatient page says so directly.

The same Medicare.gov page adds one line for Advantage members: plans "may also waive the 3-day minimum inpatient hospital stay." "May" is the word that matters. So I checked what each Mesa County plan actually filed with CMS for 2027.

What each 2027 Mesa County plan filed

CMS's 2027 plan benefit file asks every plan two questions about this. First, do you allow less than a 3-day hospital stay before skilled nursing? All 13 general plans answered yes. Second, how many inpatient days do you require instead: zero, one or two? Nine plans said zero. Aetna Medicare Signature (HMO and PPO), Aetna Medicare Eagle and Aetna Medicare Value Care said one.

A one-day requirement sounds small, and usually it is. It still means a formal admission. Two nights under observation followed by a nursing home transfer would not meet it, while it would on the nine zero-day plans. If you're comparing an Aetna plan with another plan you like, that difference is worth one question to the plan before you enroll.

What a typical rehab stay costs on these plans

Most rehab stays after a hip, knee or a bad fall run a few weeks. Here's how the filed copays add up:

  • A 14-day stay: $0 on 11 plans; $140 on Select Health Medicare Essential and HealthSpring Preferred.
  • A 28-day stay: eight days at the higher rate. That's $1,768 on most plans, $1,744 on HumanaChoice H5216-223, and $1,968 on the two $10-a-day plans.
  • The full 100 days: $17,680 on paper at $221 a day, which no one actually pays. The plan's yearly cap, $4,900 to $9,850 here, stops it first, and that cap counts all your covered medical costs for the year, including the hospital stay that came before.

For comparison, Original Medicare charged $0 for days 1–20 and $217 a day for days 21–100 in 2026. The 2027 amount hadn't been published when I wrote this; CMS usually announces it in mid-November alongside the Part A deductible. With a Medigap plan that pays skilled nursing coinsurance, such as Plan G or Plan N, that daily charge is covered by the policy.

The waiver doesn't mean automatic coverage

Dropping the 3-day rule removes one hurdle. Three others are still there on every plan in the table.

Approval. CMS's file shows all 13 plans require authorization for skilled nursing care. In practice the hospital's discharge planner sends the request and the plan decides whether you need daily skilled care, like therapy or wound care, rather than help with bathing and dressing, which no Medicare plan covers as skilled care.

The facility. The plan pays facilities it contracts with. Mesa County has seven Medicare-certified skilled nursing facilities, which I list in which Grand Junction nursing homes take Medicare for rehab. Ask the plan which ones are in its 2027 network before a planned surgery, not from the hospital bed.

Continued approval. Plans review progress and can end coverage when they decide skilled care is no longer needed. You'll get a written notice, and you can appeal quickly. Our post on how many days Medicare pays for rehab covers the 100-day limit and what happens after it.

Two plans count the days differently

Eleven of the 13 plans run skilled nursing copays by benefit period, the same way Original Medicare does: the count resets only after you've gone 60 days in a row without hospital or skilled nursing care. HealthSpring Preferred and HealthSpring True Choice filed theirs per admission instead. A second stay soon after the first starts back at day 1, which can work for you (the $0 days come back) or against you, depending on the plan's other rules. If you're on either plan and expect more than one stay, ask HealthSpring how a readmission is counted.

Special needs plans and the one exception

The rule doesn't hold for every plan sold in the county. Longevity Health Plan, an institutional special needs plan for people who already live in a nursing home, answered "no" to the shorter-stay question in CMS's file. The dual-eligible plans for people with both Medicare and Medicaid all answered "yes." If Medicaid is part of your picture, see how D-SNPs work in Grand Junction.

Where this fits in choosing a 2027 plan

Because every general plan in Mesa County waives the rule, it won't decide between them by itself. Where it matters is the bigger choice between Original Medicare and Advantage. With Original Medicare and a Medigap plan, the rehab bill is covered once you qualify, but the three inpatient days are a firm gate. With an Advantage plan, the gate is lower, but a plan reviewer and a network decide where you go and for how long. If you're weighing that trade, read whether to switch from Medigap to Advantage, and look at the hospital side in 2027 hospital copays in Grand Junction. Open Enrollment ends December 7.

How we know all this: the Medicare On Main Data Desk frames every article with public data. Here that means CMS's 2027 plan benefit and landscape files and Medicare.gov's skilled nursing and inpatient pages, all read November 5, 2026. Benefits can differ in a plan's final documents, so confirm them in its Summary of Benefits and Evidence of Coverage. Carrier names are factual reporting, not endorsement. Frequently asked questions

Do Medicare Advantage plans have to follow the 3-day rule?

No. Medicare.gov says Advantage plans "may also waive the 3-day minimum inpatient hospital stay." For 2027, every one of the 13 general plans sold in Mesa County tells CMS it accepts a shorter stay. Nine need no inpatient day at all and Aetna's four need one.

Does observation count toward the 3-day rule?

Not under Original Medicare. Medicare.gov says time under observation or in the emergency room before you're admitted doesn't count, even overnight. On an Advantage plan that waives the rule, the count matters less, but the plan still has to approve the nursing home stay.

What is the 3-day rule for Medicare skilled nursing?

Original Medicare covers a skilled nursing facility stay only after a qualifying inpatient hospital stay of at least three days in a row, counted from the day you're formally admitted and not counting the day you leave. Advantage plans can drop or shorten that requirement.

How much does rehab cost on a Medicare Advantage plan in Grand Junction?

For 2027, 11 of the 13 general Mesa County plans charge $0 a day for days 1 to 20, and Select Health Medicare Essential and HealthSpring Preferred charge $10. Days 21 to 100 cost $221 a day on 12 plans and $218 on HumanaChoice H5216-223, up to the plan's yearly limit.

Do I need approval before going to a nursing home for rehab on Medicare Advantage?

Yes. CMS's 2027 file shows all 13 general Mesa County plans require authorization for skilled nursing care. The hospital's discharge planner usually sends the request, and the facility must be one the plan will pay.

Your situation is different from someone whose plan is ending

A plan-ending member picks a replacement. Someone new to Medicare gets a one-time window: your Initial Enrollment Period is 7 months (the 3 months before your 65th-birthday month, that month, and the 3 months after), and Medigap open enrollment is the 6 months after your Part B starts — any Medicare Supplement, no health questions. Miss it and the same policy can be denied or cost more for life.
