Devoted Health Medicare Advantage Utah 2027: 5 New Counties
Most of this fall's Utah Medicare news is about carriers leaving counties. One is going the other way. Devoted Health's Utah map grows from six counties to eleven for 2027, and in three rural counties it arrives in the same year another carrier's plans end. Here is where it is, what its plans look like in the CMS file, and what a new name on the shelf does and does not tell you.
The bottom line
- 6 counties became 11. New for 2027: Utah, Weber, Duchesne, Juab, Rich. Continuing: Davis, Morgan, Salt Lake, Summit, Tooele, Wasatch.
- Three general plans, all PPOs, all at a $0 premium. In-network yearly caps of $6,350, $8,000 and $9,850.
- The two existing plans changed. Choice 001's cap rises from $5,500 to $6,350 and its drug deductible from $375 to $650.
- No Star Rating yet. CMS lists the Utah contract as too new to be measured for 2027.
- You can join between October 15 and December 7 for a January 1 start. Check your doctors and prescriptions before the premium.
A new carrier in your county is a fact, not a recommendation. I pulled every Devoted Health row for Utah out of the CMS 2026 and 2027 landscape files on October 11 and matched them by contract-plan ID. This post reports what those files show. It is not an endorsement of Devoted or a criticism of anyone else, and the file leaves out the things that usually decide whether a plan fits: the provider network, copays and drug tiers.
The five new counties
The plan counts are Medicare Advantage plans with drug coverage from all carriers, not counting Special Needs Plans. The last column is the carrier that sold in the county in 2026 and has no plan of that kind there for 2027.
| County | Plans, 2026 → 2027 | 2027 carriers | Gone from this shelf |
|---|---|---|---|
| Utah Provo, Orem, Lehi | 21 → 22 | Aetna Medicare, Devoted Health, Humana, Regence BlueCross BlueShield of Utah, Select Health, UnitedHealthcare | HealthSpring |
| Weber Ogden, Roy | 19 → 19 | Aetna Medicare, Devoted Health, Humana, Regence BlueCross BlueShield of Utah, Select Health, UnitedHealthcare | HealthSpring |
| Duchesne Roosevelt, Duchesne | 9 → 11 | Devoted Health, Humana, UnitedHealthcare | Aetna Medicare |
| Juab Nephi | 5 → 10 | Devoted Health, Humana, UnitedHealthcare | Aetna Medicare |
| Rich Garden City, Randolph | 5 → 7 | Devoted Health, Humana | Aetna Medicare |

Two different things are happening in that table.
On the Wasatch Front, Devoted is one more name on a long shelf. Utah County and Weber County each keep six carriers. HealthSpring, the former Cigna Medicare business, has no Utah Medicare Advantage plan for 2027, and Devoted's arrival holds the carrier count level. A Provo or Ogden resident has plenty to compare, and our Utah County post and our Ogden and Layton post list every plan.
In Duchesne, Juab and Rich, Devoted is a large share of the choice. These are three of the ten counties where Aetna has no plan for 2027. Each keeps the same number of carriers it had, because Devoted arrived as Aetna's plans ended. Rich County has two carriers and seven plans, every one a PPO. Nobody is moved from an ended plan into a Devoted plan. A member whose plan was discontinued chooses, and Devoted is one of the options on the list, beside the others and beside Original Medicare.

The three general plans, 2026 and 2027
Premium is the monthly plan premium on top of Part B, which you still pay. Cap is the in-network maximum out-of-pocket for the year. The drug deductible is the Part D deductible. All three plans are sold in all 11 counties with the same numbers.
| Plan | Plan ID | 2026 | 2027 |

Three things stand out.
- The caps moved up. Choice 001 goes from $5,500 to $6,350 and Choice Giveback from $9,250 to $9,850. That is the direction most Utah plans moved this year. In Salt Lake County, 18 of 24 plans had a cap under $6,350 in 2026, and 9 of 22 do for 2027.
- The drug deductibles crossed. Choice 001 goes from $375 to $650. Choice Giveback goes from $605 down to $461. If you take a brand-name drug, that line can matter more than the premium.
- "Giveback" means a Part B premium reduction. The plan pays part of your Part B premium. The landscape file does not show the amount, so I will not quote one. It is on the plan's Summary of Benefits. Look at what it sits beside: the two giveback plans carry the two highest caps. Money back each month and a higher ceiling in a bad year is a trade, and it suits some people and not others.
If you are already a Devoted member in one of the six continuing counties, your plan ID is the same and you stay enrolled unless you choose otherwise. What you received is an Annual Notice of Change. CMS counted 1,104 members in the two general plans in September, 699 of them in Salt Lake County.
The Special Needs Plans
Devoted also sells three Chronic Condition Special Needs Plans and one Dual Special Needs Plan in the same 11 counties. You must have a qualifying condition, or both Medicare and Medicaid, to join one.
| Plan | Plan ID | 2026 | 2027 |
|---|---|---|---|
| Devoted C-SNP Choice Enhanced 003 UT (PPO C-SNP), sold as Choice Premium in 2026 | H4473-003 | $37.60 · $5,800 cap · $615 drug ded. | $0 · $6,350 cap · $461 drug ded. |
| Devoted C-SNP Choice Plus 007 UT (PPO C-SNP) | H4473-007 | $37.60 · $9,250 cap · $615 drug ded. | $0 · $9,850 cap · $461 drug ded. |
| Devoted C-SNP Choice Giveback Extras 011 UT (PPO C-SNP) New | H4473-011 | Not sold | $0 · $8,000 cap · $700 drug ded. |
| Devoted Dual Choice Full 004 UT (PPO D-SNP) | H4473-004 | $34.40 · $9,250 cap · $615 drug ded. | $0 · $9,850 cap · $700 drug ded. |

What a new carrier cannot tell you yet
There is no Star Rating. CMS published 2027 ratings on October 8, and Devoted's Utah contract, H4473, is listed as too new to be measured (CMS Star Ratings data tables). That is a statement about time, not quality. Ratings are built from prior-year performance data that a new contract does not have yet. Our Utah Star Ratings post shows the rated contracts.
There is no local track record in the five new counties. A plan that is new to Roosevelt or Nephi has a network that was built this year. So the network is the first thing to check, and you check it by name:
- Your primary care doctor and every specialist. Search the plan's 2027 provider directory, then call the office and ask, "Are you in network for this plan for 2027?" Use the plan name and number.
- Your hospital. In a rural county that usually means one local hospital and one referral hospital on the Wasatch Front. Check both.
- Your prescriptions and your pharmacy. Run your list through Medicare Plan Compare, which prices each drug on each plan.
These are PPOs, so you can generally use out-of-network providers who agree to see you, at a higher cost and under a higher combined cap. That flexibility is real, and it is not the same as being in network. Our Salt Lake County PPO post lists every 2027 PPO there.
A new plan name showed up in your county. Is it a fit?
Brian Penner has worked with Medicare clients for more than 22 years and is licensed in Utah. Bring your doctor list, your prescriptions and the card you carry now, and we will check the plans we offer in your county against them and price Original Medicare with a Medigap policy beside them. No cost, no pressure. We do not offer every plan available in your area.

Members of a discontinued plan in Duchesne, Juab, Rich, Utah or Weber County have the same three doors as everyone else in that position, and more time than most people realize.
- Another Medicare Advantage plan in your county, chosen between October 15 and December 7 so it starts January 1 (Medicare.gov).
- Original Medicare with a Medigap policy and a stand-alone drug plan. Because your plan was discontinued, you can buy Medigap Plan A, B, D, G, K or L with no health questions if you return to Original Medicare, from November 1, 2026 through March 4, 2027 (Medicare.gov). Medigap has no network. In a county with two or three carriers, that is worth pricing before you decide.
- Original Medicare with a drug plan and no Medigap. The lowest monthly cost, and no yearly limit on your 20% share.
A Special Enrollment Period also runs from December 8 through February 28 (42 CFR § 422.62). How long you have, date by date. The Medigap right belongs only to people whose plan ended. If you are simply curious about a new plan and your current one continues, switching is an ordinary fall decision, and going back to Medigap later may involve health questions.
Where Devoted is not
Devoted has no 2027 plan in St. George, Cedar City, Logan, Brigham City or Price, and none in southeast Utah. Grand and San Juan counties are not on its map in either year. Our statewide list of Utah plan changes covers every carrier and county.
How we know all this: the Medicare On Main Data Desk frames every article with public data — here, the CMS CY2026 and CY2027 landscape files compared by Contract Plan ID on October 11, 2026, the CMS September 2026 enrollment file by contract, plan and county, the CMS 2027 Star Ratings data tables published October 8, 2026, and the federal rules and Medicare.gov pages on enrollment periods and Medigap guaranteed-issue rights. Enrollment cells under 11 are suppressed by CMS and are not in our totals. The landscape file does not show copays, drug tiers, provider networks or the amount of any Part B premium reduction; the plan's own documents do. Medicare On Main is a licensed independent insurance agency. We do not offer every plan available in your area. Any information we provide is limited to the plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options. Not connected with or endorsed by the U.S. government or the federal Medicare program. Mentioning a carrier is not an endorsement or a criticism; plan availability is a fact of the market. This is education, not advice; confirm your plan, costs and eligibility with a licensed agent or Medicare.gov.

Yes, in 11 counties. The CMS 2027 landscape file lists Devoted Health plans in Davis, Duchesne, Juab, Morgan, Rich, Salt Lake, Summit, Tooele, Utah, Wasatch and Weber counties. In 2026 it sold in 6. Is Devoted Health available in Utah County for 2027?
Yes. Utah County, including Provo, Orem and Lehi, is one of the five counties Devoted Health added for 2027. All three of its general PPO plans with drug coverage are listed there at a $0 monthly premium, with in-network yearly caps of $6,350, $8,000 and $9,850. Utah County has 22 Medicare Advantage plans with drug coverage from six carriers for 2027.
Which Devoted Health plans are offered in Utah for 2027?
Three general PPO plans with drug coverage: Devoted Choice 001 ($0 premium, $6,350 in-network cap, $650 drug deductible), Devoted Choice Giveback 002 ($0, $9,850, $461) and the new Devoted Choice Giveback Extras 014 ($0, $8,000, $700). There are also three Chronic Condition Special Needs Plans and one Dual Special Needs Plan. Every plan is sold in all 11 counties.
What is Devoted Health's Medicare Star Rating in Utah?
It does not have one yet. In the 2027 Star Ratings CMS published on October 8, 2026, Devoted's Utah contract, H4473, is listed as too new to be measured. That means there is not enough history for a rating. It is neither a good score nor a poor one.
Is a $0 premium Medicare Advantage plan free?
No. A $0 plan premium means the plan charges nothing on top of your Part B premium, which you still pay. You also pay copays and coinsurance when you use care, up to the plan's yearly in-network cap. On Devoted's three general Utah plans for 2027 that cap is $6,350, $8,000 or $9,850.
Does Medicare On Main charge to compare plans in my Utah county?
No. Brian Penner is an independent licensed Medicare advisor with 22+ years of experience, licensed in Utah and paid by the carriers, not by you. We do not offer every plan available in your area, and we will tell you plainly when Medicare.gov or 1-800-MEDICARE is the right next stop.
